Appointment Policy:

When you make an appointment, we will ask for a Visa or MasterCard number to guarantee your appointment time.

Cancellation Policy:

We work hard to accommodate your needs and we ask for a 24 hour advanced notice if you need to cancel or reschedule your appointment. This enables us to be available to help other clients. As is customary, failure to provide notice within the specified time will result in a billing of 50% of the scheduled service. Canceling or rescheduling your appointment 5 hours or less before the scheduled time or missing your appointment entirely, will result in a full billing of the scheduled service.

Payment Policy:

We accept payment in the form of cash, checks, VISA or MasterCard.

Insurance Policy:

We will bill Personal Injury and Worker's Compensation claims for clients with the following information:

1. Insurance Claim Number and Approval Date
2. Prescription from Doctor
3. Date of Injury
4. Date of Birth and Social Security Number
5. Insurance Carrier Name, Phone Number and Agent’s Name
